Chemotherapy-induced peripheral neuropathy (CIPN) represents a significant and increasingly prevalent complication arising from modern cancer therapies, directly affecting the peripheral nervous system. As advancements in oncology lead to improved patient survival and extended treatment durations, the clinical burden of CIPN continues to escalate, impacting a substantial proportion of individuals undergoing chemotherapy.
The neuropathic manifestations of CIPN are diverse and debilitating, primarily stemming from the degeneration of peripheral sensory, motor, and autonomic nerve fibers. This damage leads to a spectrum of symptoms including profound sensory disturbances such as numbness, paresthesias, and pain; motor impairments like muscle weakness, balance deficits, and gait unsteadiness; and various autonomic dysfunctions affecting cardiovascular, gastrointestinal, urinary, and sudomotor systems.
Symptoms typically emerge in a characteristic "glove and stocking" distribution, originating in the extremities and potentially spreading proximally. The onset can vary from immediate (hours to days) to delayed (weeks to months) after chemotherapy initiation. A particularly challenging aspect is the "coasting effect," where symptoms can progress or even worsen for months to years following treatment cessation, often becoming irreversible in chronic cases. This chronic neuropathy profoundly diminishes patients' quality of life, compromises their functional independence, and elevates risks such as falls and psychosocial distress.
Despite its high prevalence and significant impact, there remains no standardized preventative or curative treatment for CIPN, underscoring an urgent unmet medical need. Current management strategies are largely focused on symptomatic alleviation and supportive therapies.
